Gift Vouchers: With gift vouchers the reference number of the gift voucher must be given in when making the appointment and also the gift vouchers must be handed in first before treatment. Treatments can not be done without a gift voucher, otherwise full payment will be required.

VIP CARDS: Can only be used Monday to Friday for treatmenrts & gift voucher treatments. Vip Cards cannot be used in conjunction with any other offer or promotion. Vip Cards remain the property of relax & can be cancelled at any time.

Terms and Conditions:

This is a Legal Document that affects your rights. 

This agreement is for participating in Group Fitness Training. The Trainer refers to the Personal Trainer/Group Trainer. The Activity refers to the participation in personal/group training.  

• I agree to eat a meal at least 1 and a half hours before every Training session. 
• I agree to bring a drink bottle, towel and also any medicines that I might need to take if needed. 
• I understand that I must make alternative arrangements for my children & my babies care. 
• All clients must be weighed at the start and at the end of the 5-week training workout. 
• I understand that if I put on weight during or after the 5week workout is over, I will not hold my trainer responsible. 
• 5 week training workout must be paid for, in full before start date.  
• Late arrival will of course mean a shorter session. 
• I understand that the 10 sessions will run twice a week Monday & Thursday for 5 weeks only. 
• I understand that any money paid will not be refunded even if I have changed my mind or need to cancel all my sessions or miss any sessions. 
• I acknowledge that it is a condition of participating in this activity that I do so on my own risk. 
• I accept all risks and hereby indemnify and release the trainer, their agent, affiliates, employees, members, and any person or body directly and indirectly associated with the trainer, against all liability (including liability for their negligence and the negligence of others) claims, demands and proceedings arising out of or connected with my participation in this activity. 
• This release and indemnity continues forever and binds my heirs, successors, executors, personal representatives and assigns. 
• I acknowledge that participating in this sort of activity may involve injury or even death. 
• I recognize the difficulties associated with the activity and attest I’m physically fit to participate safely in the activity and that a qualified medical practitioner has not advised me otherwise. 
• I understand the demanding physical nature of this activity. I’m not aware of any medical conditions, injury or impairment that may be detrimental to my health, if I participate in this activity my Trainer will be immediately informed. By continuing to participate in this activity, I accept the risk despite these conditions and I’m still and will always be under the terms of this agreement. 
• I certify that I’m 18 years or older and have read this document and fully understand it and agree to all the terms and conditions

OR  

·         As a parent or guardian of the participant (a) I agree to the above for myself and on the behalf of the participant and (b) I indemnify and will keep indemnified any person or body directly or indirectly with the conduct of the activity on the terms and conditions referred to by Relax Fitness Club.
